Questions and resources related to Hormone Replacement Therapy (HRT), for transgender and GNC individuals

RULES

  • Moderators may take action or inaction at their discretion outside of these rules if needed, although we do generally try to stick to them.

  • Be kind to yourself and other users
Hate speech is prohibited. No racism, homophobia, transphobia, and so on for all intrinsic characteristics. Do not personally attack other users. This includes ironic self deprecation, transmedicalism advocacy, calling people's "transness" into question, and similar lines of comments. If you think another user has said something incorrect, reply politely or walk away.

  • Stay on topic
We're a subreddit intended to help people with HRT and adjacent medical issues. Social posting and non-medical transition questions should go to more generic trans subreddits such as /r/MtF, /r/FtM, /r/AskTransgender and so on. For non-HRT related medical issues you should be talking to a doctor.

  • Be clear, be accurate

  • When asking questions, be clear about what the problem you're facing is and provide clarifying information when requested. Post titles should be informative but brief. Ask for help with the root problem rather then what you think the solution might be wherever reasonable (https://en.wikipedia.org/wiki/XY_problem). Confirm answers with multiple people wherever possible, because sometimes people will say dumb stuff and it doesn't get caught in the moment.
  • When answering questions, stick to established science wherever possible and have sources for your statements. If you can stick to peer reviewed published work that doesn't have major critiques then perfect, if this doesn't exist then use community resources which have stood up to scrutiny. Anecdotes are generally not sufficient.
  • Keep discussions in public
Do not DM other users. Do not ask other users to DM you. Report and block people who DM you, especially if they are trying to sell you anything. Keeping discussions in public is the primary tool for avoiding misinformation and scams.

  • Prohibited/restricted topics
Some topics are prohibited, either due to reddit site wide ToS issues, because they are effectively unsolvable, or they have a tendency to generate high quantities of rule breaking posts.
Some topics are restricted, and posts will be removed unless they are of high quality. You might want to poke a mod before posting to check if this is the case.

  • THE PROHIBITED TOPICS LIST:

    • Sourcing for Testosterone (Reddit ToS, https://support.reddithelp.com/hc/en-us/articles/360043513471)
    • Circumventing the law in your location (Reddit ToS)
    • Direct advertisements from storefronts or other sellers (Reddit ToS, drama)
    • Direct transactions between users, including giveaways (Reddit ToS)
    • Asking for medication advice while under 18 (Reddit ToS), see wiki for more information
    • Hiding medication/transition from family (no good answer, drama)

    Restricted topics:

    • Advocacy for non-pharmacutical medication (IE, herbals and phytoestrogens)
    • Discussions around medications only tangentially related to transitioning (Pioglitazone, Semaglutide, and similar)
    • Requests for participants in research will be removed unless prior moderator authorisation is sought.
    • Requests for interview from journalists will be removed without prior moderator authorisation, and in the current environment authorisation is unlikely to be given.

Reviews and recommendations of storefronts or suppliers
Second party reviews and recommendations are allowed in moderation. Excessive or unprompted promotion will lead to post removal. Requests for storefronts or suppliers are also permitted within reason. Storefronts/suppliers may not self-promote. Paid incentives to promote are prohibited.

    Frequency/necessity of bloodwork

I live in an area where access to frequent (or any) bloodwork is a significant hassle and costs a lot, is there any way of skirting around it or do you need to just bite the bullet?

    Dosage and Frequency of Bicalutamide

So, we know that Bica has a half life of 4 to 6 days. Typical dosage and frequency is established as 50mg/day - but for cis men and cancer patients.

Given that trans women have already typically lower levels of T (especially on E monotherapy), it should be enough to take Bica once weekly? (At most twice weekly to be on the lower range of half life).

(To answer why Bica on Monotherapy: Bica blocks action of DHT, which can still be synthesized in high levels even with lower T, and with some even from Progesterone. And while Finasterid blocks T->DHT, it does not blockt Prog->DHT. And in contrast to Bica, both Finasteride but especially Duta block all action of 5ARD, like the good stuff from Allopregnanolone, which we actually want.)

Moving to the UK, where & how do i diy

i’m going to the uk soon and will stay there as a student for a couple of years i was wondering how easy it is to start to diy there, how soon will i get my Estrogen + how much do i have to buy at once and from where do i buy it? are there physical places i can go too, or some websites withn the uk, or outside the uk that can deliver fast? im also on Pills rn, i assume going on injections or gel would be better. for the injections where do i get the injections from? i assume ill only buy the estrogen vials and i have to get the needles separately. how many vials do i buy at once? ive had it easy to diy where im from so im scared going to the uk will mean there will be some complications and i have to stay off of E for a while which i dont want to happen. please help me with knowing where to get it from and how the process works overall

    Spironolactone and high DHEA-S.

Does spironolactone helps negate effects of elevated DHEA-S? If yes at what dosages?

Is trans diy a bad thing to do if you want to go private or NHS at somepoint? (Ftm)

I'm really considering trans diy right now (ftm) just using t gel, but can this cause problems with the nhs (when I get referred) or going private when I can afford it?

I just dont want to mess up anything to badly, for when I can get referred on the NHS or afford private etc.

    Thank you

    Question about blood tests

Are blood tests absolutely necessary if you're unable to get them in your situation? Would it be safe to just stay at a low amount until you're able to? And if so, what amount? Also, do you need blood tests for t blockers?

339pg/ml, 24ng/dl after two weekly injections, compared to..

92.5 / 150 after 4.5 months through planned parenthood, stuck on 1mg E and 50mg spiro twice a day. the kicker? they told me that was fine and changed nothing about my treatment plan. switched to DIY and went from crying in despair over learning how abysmal my levels were, to crying in joy at finally meeting the image i had for myself

Hi!, Im from Peru and i’ve been using estradiol valerate pills for 3 years, and now I’m considering switching to injections. Has anyone had experience with Otokonoko, Astrovials, or other online stores for this? I’d love to hear about any experience. Thanks

Has Testosterone HRT triggered anything like Endometriosis for anyone?

Hi, I'm (M22) have been on Testosterone DIY for nearly 2 years. Going well, Im lucky to pass easily in dad to day life and noone clues a thing.

But a price I've had to pay is what I imagine the start of labour must feel like when having contractions. It started over a year ago now and it's only getting worse, almost daily, can be triggered simply by standing up. I'm on a wait list for GIC, estimated 2-6 years and my blood tests for my hormone levels don't indicate anything abnormal, and I've been on a set dose for months now.

My GP is currently being slow with investigating the issue, but to be fair to them, I have a fair few issues. Fibromyalgia being one, and that can cause contraction like pelvic cramps. It could also be vaginal atrophy of vaginismus but I don't fully meet Symptom criteria for either. Other thing I suspect it could be is Endometriosis. TMI AHEAD. (pain paired with the fact that penetration of any kind is painful, and I cannot understand why or how anyone could enjoy sex when it's that uncomfortable.)

I don't know what I'm trying to ask, but I feel very lost and dysphoric with this getting worse, not only that, the pain is worse than almost all periods I've had. It's renders me immobile, catatonic and mentally paralysed. It feels like if it gets any worse it will kill me. A few times it's made me pass out. That isn't normal, even I, a clueless clumsy dope, know that. I've only recently finally been prescribed pain medication but it's still not touching it. Testosterone may have cured my PMDD, hugely improved my self image, dysphoria and life, but I think it's started a war with my ovaries. I cannot live laugh love in these conditions.

Does anyone have/had any similar experiences they'd be willing to share? How did you deal with it? How do I navigate medical side of things? Thanks in advance :)

    Needle questions

For gluteus IM injecting of Estradiol Valerate, which has been your more successful size of needles. 1 inch, 1.5 inches,..,? I’ve used 25g 1.5 inches (luer lock) but I’m wondering if I can also get the same effect with the 1 inch or just keep using 1.5. What sizes have you liked using for glute IM?

    MTF Dosage Recommendations

Hey I was hoping to get some help because my doctor refuses to help (shocker I know). I have been on Estradiol Valerate for 3 months, my dose is .4ml at a 50mg/5ml once a week on Saturday nights. I am also on 25mg of Spiro daily.

Per my doctors instructions I did my labs 5 days after my injection (I did them Saturday night and got my labs done Thursday morning). Well I just got them back and I am wondering if yall have recommendations for increasing my dosage. I am currently at 216 pg/ml for Estradiol and 18ng/dl for testosterone.

My goal is to be between 300-400 pg/ml for my Estradiol but since my doctor won't support that I need to increase my dosing on my own which is why I am coming here to ask for advice on what to increase it too or if it would be better to just dose more frequently, any suggestions or advice would be super appreciated.

Question about my dosage after blood results

    Hello,

I have a question about adjusting my dosage because my blood test results came back higher than expected. I did a lot of research before going the DIY route, but it seems my body is reacting differently than anticipated.

    Here’s my dosing history:

    • 02.04.2024 – 02.09.2024: 6mg (0.15ml) of 40mg/ml Estradiol Enanthate every 7 days
    • 09.09.2024 – 20.01.2025: 6mg (0.12ml) of 50mg/ml Estradiol Enanthate every 7 days
    • 27.01.2025: 5mg (0.1ml) of 50mg/ml Estradiol Enanthate every 7 days ( It may not have been the best idea to start reducing the dosage before my blood test on Friday )

    Blood Test Results:

    DateTestosteroneEstradiol (E2)SHBGProlactin
    09.08.20240.18 ng/ml472 pg/ml94.6 nmol/l-
    31.01.20250.15 ng/ml664 pg/ml103 nmol/l18.9 µg/l

I was quite surprised by how much my estradiol levels increased, so I’d like to reduce my dosage. I'm considering lowering it to either 5mg or 4mg every 7 days.

Would this be a good approach? Any advice would be greatly appreciated.

    Thanks in advance!

Lily Alexandre's video on bica and male pattern baldness

So, recently I've watched Lily Alexandre's video about DIY HRT and something called my attention. She mentioned that Bicalutamide doesn't prevent male pattern baldness, which is something I have never heard and that even sounds contradictory considering that it is sometimes used to treat baldness in cisgender men and women. Does anyone have any resources that can be consulted or have heard about this?

    Splitting Bicalutamide?

Been kind of curious about this, I noticed that my Bicalutamide pills have a "Don't Cut" label on the box, I've been cutting mine for awhile now because that's how I was original recommended I do it, but I'm wondering about if it's okay to cut them now, if it's something I should be concerned about?
