(Demographic) - Who you want to answer your survey; age, country, characteristics, etc. Let potential respondents know whether they should participate (required even if your survey is open to everybody).

When asking about gender or sex, include "another gender" and/or a "fill in the blank" option if your demographic is for (Everyone).
This helps with data integrity, and it also helps with inclusion. More people than you might think would fill out "other" on a gender question! If you are strictly interested in only people who identify as Male or Female, make sure you use (M/F) in your demographic.
If you are asking about race and/or ethnicity in your survey, please know the differences between them.
Race refers to physical characteristics that one shares with other people of one's race, while ethnicity refers to one's culture and traditions. If you mix these up, you once again may run into data integrity issues!
Don't forget to consider ethical implications and consent for your research.
If you require signed consent from a moderator before posting your survey, feel free to message the moderators and we would be happy to help you out.
Please refrain from soliciting participants in the comments section of other surveys.
The discussion section for each thread is for comments about that survey. If you see people doing this, feel free to report the comment, and we'll remove it and talk to them about it.

For my thesis, I created a survey to assess the prevalence of those at risk for diabulimia, as well as to assess diabetes management, eating habits, and insulin habits in adults. Description:
This study will investigate how the use of different porn formats, reasons for using porn, and sexual fantasies relate to each other. Participants will be given surveys through Qualtrics to measure how often they consume different porn formats, how they feel about different sexual fantasies, and why they use porn.
If you choose to participate in this study, you will complete an online, cloud-based survey using the Qualtrics Survey platform. You will be asked to answer a series of questions including demographic information regarding your pornography consumption habits, pornography consumption motivations, and atypical sexual fantasies. If you do not feel comfortable with the areas of focus or content (i.e., pornography use, atypical sexual fantasies) of this study, please do not participate. If you do participate, remember that you are not required to answer any question that you do not wish to respond to.
The survey is 86 questions long and should take approximately 30 minutes to complete. You can leave the survey and return to it as needed.
You will have the opportunity to win one of four $25 (CAD) gift cards. After you have submitted your responses to the survey, you will see a hotlink you must click in order to be entered into the draw. You must provide a means of contact (e.g., email) and complete a skill testing question in order to be entered. The information you provide cannot be connected back to you in any way; it is only used to enter you into the Amazon Canada gift card draw.
To participate, you must be fluent in English (English level Grade 12) and be at least 18 years of age.
