Here's a suggested format but feel free to do it your own way:

Appearance: 5/5 What color is it? What color is the light that refracts through? How thick is the head and how long does it last? Does it leave lacing on the glass?

Aroma: 5/5 What does the beer smell like? Fruit? Malt? Coffee?

Palate: 5/5 How does it feel? Does it bite like an IPA? Is it soft and velvety like a heavy stout? Does it dry your mouth as it goes down?

Taste: 5/5 How many different flavors can you detect? How do the flavors change over time? Are there citrusy overtones to the hops? Is it sweet? Does it taste nutty or bready? Was the flavor what you expected based on the aroma?

Overall 20/20 Sum everything up. Would you get this beer again and would you recommend it to others?
