I have been deep in the weeds of TTC PCOS for a while, learning SO much from all of you. I have had a PCOS Diagnosis since puberty. I have an appointment scheduled with an fertility doctor (CCRM) in mid December. I was excited to potentially start letrozole after reading through many of your success stories! I am also doing all the common early interventions... metformin, letrozole, low carb, trying to track ovulation with an Oura ring...
I was filling out my intake forms and looking closely at my last labs when I noticed my TSH was at 8.280 on October 21st. My previous test was June 12 and it was .659. I have been medicated for Hypothyroid for at least 10 years and am on a relatively high dose of Levothyroxin 150MCG. I reached out to my PCP and she suggested that it could be an absorption issue, and to take the Levo right before bed instead of before breakfast. I will start this right away.
Not asking for medical advice... just asking:
Will the fertility Doctor have special expertise in managing my Hypothyroid or should I immediately look into getting in with a different kind of doctor? Posting here because I know PCOS and Hypothyroid often go hand in hand.
A podcast I listened to this morning talked a lot about dairy free and gluten free being important. I am very overwhelmed now trying to combine PCOS guidance with the Hypothyroid guidance.
I have been so consumed in managing my PCOS for the last 6 months of TTC, I haven't really given much thought to the Thyroid issues and now really feel like I have dropped the ball somewhere. I have had the Hypothyroid diagnosis since I was in college, and never even knew that it had fertility impacts.
I appreciate any feedback! This subreddit has been such a great resource since starting TTC. <3
Hi all! I am 30F with PCOS (I suspect lean since 5’4 and 125lbs) Testosterone is very elevated. AMH is 13 or so last it was checked. Irregular cycles for as long as I could remember. We’ve been TTC for over a year. I started with clomid at a low dose (can’t remember exactly) but no response. Then I tried letrozole 5mg but had no response then finally at 10mg of letrozole I would have 32 day cycles. (3 cycles of that) My doctor is suspicious because my ovulation would seem that it’s around day 14 and then I’d have a prolonged luteal phase. This past cycle I took 10mg for 5 days as I always do but it was monitored they confirmed one follicle at 22mm or so and we did a trigger shot and iui. Lining looked great but no success. I think ovulation was around day 16 and cycle was 31 days. I also started 500 mg ER metformin with this last cycle. Doctor thinks I should move on to IVF because he thinks if this was working appropriately I would have been pregnant already. Husband - great SA and they did check my tubes and it was clear. I’m feeling very defeated. Not sure if I should bite the bullet and do ivf or if there’s another way. We do want more than one child God willing.
Does anyone else in this group have borderline personality disorder? I’m starting to feel like the combination of PCOS and BPD is a rare one and it would be great to speak to others who know how it feels.
Second Letrezole cycle (7.5mg) but no trigger or monitoring for this cycle. CD15 I ran out of OPK's but my FMU was a .93 so I assume I got a positive that day. The next morning, CD16, I woke up to INTENSE cramping. I've only had csections but I feel like this is what contractions must feel like. I tried to use the restroom and it was like an electrical shock to my anus whenever I tried to push. I gave up because it felt like what I assume giving birth through your butt would be like.
I looked at some studies and see that cramping doesn't mean you're ovulating that second, but that cramping can happen during ovulation. Based off the near positive OPK the day before do you think the cramping signaled ovulation? It was intense for about 8 hours and then just random throughout the day. I also considered it could be a cyst that formed from late ovulation (I had a 2.6cm follicle on CD14 the previous cycle) and maybe it burst? Or maybe multiple follicles were mature and released? (I see studies suggesting without trigger though only the lead would release on a regular Letrozole cycle?)

I have been seeing an RE for 3 months now after being diagnosed with lean PCOS. I had a lot of hope originally, but yesterday that dropped.
My first month on letrozole 2.5mg, my follicles didn’t grow. I was put on provera. Month 2 - I had an active cyst (presumably because I didn’t ovulate). I waited a week from my baseline, and they saw it was decreasing, so I was prescribed 5mg. Bloodwork showed I ovulated but I am convinced this wasn’t a normal cycle because of the cyst. Month 3 - I took 5mg again, but no trigger shot because my LH was surging.
I received a call yesterday and my RE asked if I was ready to switch to IVF?? For someone with regular periods (despite PCOS) I was shocked by this. I’m turning 33, but I still feel like I have some time? My husband and I have been wanting TI to work, but do they know something I don’t know? All my tests came back clear minus the PCOS.
How many months on letrozole is standard? I feel like I ovulated successfully for the first time the last round, but I’d least like to try a trigger shot or something else before giving up on TI. Please help!

My cycle ended two weeks ago and now I have randomly started having brown discharge for three days now. It’s only visible when I wipe it’s not a lot. This is supposed to be my ovulation week just curious about what it could mean.
Also, side rant… my VERY fertile mother is telling me I’m being too anxious about getting pregnant. I’m i’m upset because I know that I’m not being that anxious about it. I’ve really been controlling my thoughts and I’ve been working on my anxiety around it however, I also know that I have a reason to be a little concerned. I’m on medication that’s a steroid for my digestive system because I was I was told by the doctor I had some colitis symptoms or some colitis issues when I did my colonoscopy this year so they were giving me a steroid to clear it up and apparently I won’t have to take the medication anymore after I do the 90 days and they said that after I finished the medication that it should be cleared up, which I don’t even know if that’s true because I had a flareup when I didn’t even take it for a day so very nervous about that and I know that digestive stuff really affects our hormones so it’s just a lot of of unswered questions that I’m concerned about. The problem with the steroid is that I’m checking my ovulation levels and they have been low since using in the steroid. Has anybody had any issues that relate to this or that have taken steroids before and affected their ovulation?

Just going to rant here because I am exhausted. We have been trying for over a year now. Got the bfp twice, once a year ago(ectopic) and then again 6 months ago(chemical). Both these times it was our first month trying so when we started again 2 months ago we were positive that we will be getting a positive soon. Both the previous times i had underlying health issues like untreated thyroiditis.
We were happy, all the issues were under control, both me and my husband’s tests were normal. I know I ovulated every month. My cycles are a little longer around 33 days but they are regular, I do normally start spotting 5 days before the period begins. The only reason we were advised letrozole, trigger and progesterone was because of the 1st two mishaps.
1st month on 2.5mg letrozole and I ovulated on day 18,later then i normally do. The lining was 11.5mm and the follicle was 21mm. I took progesterone suppositories twice a day but started spotting followed by a bfn.
2nd month now, took the same 2.5mg letrozole, day 3-7 and on the day 10th ultrasound one follicle was already 20mm and the lining was 7.5mm, i was told to trigger the same day and i think i ovulated on the 12th day in the morning. Today is 7dpo and I had some spotting. In my mind I am just out of this cycle.
On both tries we did the bd one day before the trigger and continued 2 days after the trigger.
I am so exhausted getting hopeful every time and loosing it again after some days. We are not old 32(m) and 29(f).
I have lost 25lbs through a low carb diet and exercise. I decided to try with Letrozole again so I made an appointment for today to see the fertility dr. I made this appointment 8 weeks ago. At the very last minute (I was about to head out the door) they send me a text that said due to unforeseen circumstances they were cancelling my appointment and on a later day they will message me to reschedule. This is very on brand for this clinic. They've canceled other appointments and are chronically very late. In Canada we have government funded IVF but you have to be on a waitlist. I've considered switching clinics but you start over if you move and the wait lists are over two years long! So if the letrozole does not work I am so close to doing IVF. I unfortunately can't pay out of pocket currently so this is my only option. My heart is breaking over how poor the communication has been in a very stressful journey. I don't even know if Letrozole will work for me.
I take inositol and metformin along with omega 3, NAC, Zinc and B complex vitamins.
I eat two low carb meals a day and have two "servings" of carbs at dinner as per recommendation of a fertility nutritionist. I have done a 180 in my eating since seeing her.
I was so hopeful that I was getting a period since I have some brown blood in my discharge Sunday and yesterday but it stopped today. I thought I ovulated 11/3 because I got a ton of egg white mucous. I have not been tracking other methods because it would have cost a fortune as I wasn't being monitored at the time. I was hoping to ask for a blood test for progesterone while I was there to know for sure.
TWINS QUESTION:
If you were open to twins & your specialist was trying to have 2-3 mature follicles to release at trigger to up your chances, what happens if 1 follicle is mature & releases in a natural ovulation & then you trigger the last 2 that were close behind a day later?
Does that then release the other 2 for an extra chance at conception?
Basically I had 1 follicle that was 21mm & 2 follicles that were 15mm & 17mm & my lining was a little thing only just above a 6.5 so they were waiting for a couple days for my lining to thicken & my follicles to grow but then I did a ovulation test this morning & it showed peak smiley & I also have sticky egg white consistency CM so now I’m worried I have already ovulated early…
I go in for a scan tomorrow but if I had ovulated could you trigger still tomorrow & release the remaining smaller follicles?

I need a saline sonohysterogram (SIS) to better visualize a suspected uterine polyp. My periods have 0% predictably to them. I could have a 30 day cycle or a 200 day cycle (eventually I’d call for progestin to cause a withdrawal bleed, but you get it). I need to get this polyp settled before we figure out ovulation induction. If I magically happen to be ovulating (I track OPKs and CM) at the time of the SIS, would I need to reschedule it? And then if a polyp is confirmed, I would need a separate outpatient procedure to remove it. Same question for that too - reschedule if ovulation rears its head? Obgyn appts are only ever 2-3 months out at any given moment, so I’d hate to further delay everything. At the same time, I also hate the thought of missing an opportunity to conceive. I could be overthinking this for nothing…who knows if I’d even be nearing ovulation for either procedure lol. We’re almost one year in and I’ve ovulated (allegedly) 6x in that year, so 🫠
Hello! I’d really like advice from anyone that’s been in a similar situation - I just had a canceled IUI cycle due to over stimulation from 5mg letrozole followed by 75 IU follistim. I have 7 follicles 16-18mm. I’d like to add that this is my 1st medicated cycle since starting Metformin and having a hysteroscopy to remove a polyp. Prior to that I’ve had multiple letrozole rounds with the following outcomes-
As you can see, my response to these meds is all over the place. My doctor wants to me go back to just letrozole 5mg or try clomid next. Has anyone gone through this? What did you do? I was really hoping to have this IUI before the holiday season which I’m thoroughly dreading. I’m devastated.

My partner (28M) and I (25F) have just been successfully referred for IVF with ICSI treatment starting in May/June 2025.
I have PCOS, with AFC 36 on one ovary and AFC 34 on the other, everything else was healthy and normal. My AMH was 54 pmol, and my fertility specialist has warned me I would be at higher risk of OHSS.
My partner has a low sperm count (only 10 million), 41% motility and 3% morphology but our specialist wasn’t too concerned about the last two factors.
Any advice, success stories, or discussions are welcome! I want to know similar stories etc. I want to be as motivated and educated as possible going into our first cycle!
Without going into my whole life story, I've been trying for my 2nd for years. My current (and fantastic) doctor diagnosed me with PCOS and started me on Letrozole (increasing the dose after the first month) and now also has me on estrogen and progesterone every evening (first cycle). I've been testing using a combination of easy@home and Premom strips which have pinpointed my LH surge very clearly for the past 4 months. Temp tracking has also supported ovulation. My doctor said that she recommends getting the Mira, but it is A LOT of money, even with the discount code she gives. I know it tracks estrogen and progesterone as well, but still. Is it ACTUALLY worth it?? I don't wanna spend upwards of $200+ and be stuck with a subpar product and still no baby haha.
No hate, I've been trying for years and I'm desperate and simply grasping at straws for advice. TIA.

It's been a month that I am on metformin I hav gastritis but metformin made it worsen. I am taking 750 mg once a day doc told me I can't drop the metformin as I have this recurrent miscarriage history. wht should I do Is there any other alternatives instead of metformin?
