Male fertility is a spectrum. All pain is legitimate. All of our struggles and frustrations are real. We will not tolerate piling on people who's infertility is numerically "less severe" than others.

Virtually without exception, everyone who is new here is here because they have discovered that male fertility is not a matter of having or not having sperm. Assume those posting new threads are scared and/or overwhelmed and provide support accordingly.
--Mentions of success are allowed.
Success stories? We want them! SA improvements, egg retrievals, even positive pregnancy tests. If you've been through the infertility journey and have good news, POST IT.
--Understand that not everyone is a Google scholar. The internet has a staggering amount of information on it. Far more than many people can sort through and process. Even the sub's SA wiki can be intimidating, especially to people new to their infertility whose brains are still swirling with anxiety. Rudeness will not be tolerated. If you're unwilling or unable to be helpful, just move on.

Hi I have NOA and I am opting for mtese does anyone have side effects after the surgery like ED, Hypogonadism or any other and also I have borderline low T. Please let me know who have already experienced it.
My husband has non obstructive azoospermia and we recently did an extended sperm search where they search the sperm sample for hours and got news they found 0 sperm.. was expecting it but still hurts not even 1 sperm found.. we have microTESE surgery booked end of November and that is our only option.
Any success stories with high fsh as well?
Im making this post to 1. Get some of you guys opinion on my situation and 2. To hopefully help someone else who see this in the future.
2019 I did semen analysis (I was 29 then) and I had low motility. I never followed up with anything for reasons that’s not relevant to this thread.
Now I’m 34 and I had bloodwork done and I’m told I have an hormonal Imbalance.
My estradiol is 45H
Total testosterone is 320
Free testosterone is 58.7
FSH 2.1 LH 1.5
So apparently my body is producing too much estrogen which is possibly affecting my fertility and also some thinning hair unfortunately. I did a semen analysis last week and will see my results at my doctors visit on Friday 11/1/24 and I expect to get prescribed Clomid. I started a meal plan, and supplements of fish oil, COQ10, ashwgwanda, vitamin D3, triple maca, K2 MK-7, Boron, vitamin C
Anybody else experience this? Any advice?
I'm 32 and my wife 42. We decided to start a family, in order to save time I did a seminigram. I'm really worried about the results, I'd like at least to be a candidate for ivf. Those are some of my results: Viscosity: high Volume: 8,3 Concentration: 7.300.000 Total count: 61.300.000 Quick progressive movility: 15% Immobility: 80% Normal morfology: 4% Normal Kruger morfology: 3%
My lifestyle is not so healthy, I always sleep less than 6 hours. I'll start to improve that from today. I'm fit, I workout a lot, but I eat tons of snacks and just a few vegetables and fruits. I'll fix that soon.
My husband had a bilateral varicocele embolization yesterday, while under they did a biopsy and found no sperm. We’re so disappointed. Riding the infertility train of highs and lows has been exhausting. His hormone levels are normal except FSH, which is just slightly above normal range. The doctors have had hope that the varicocele have been pushing a low count to 0. I’m wondering if anyone would be willing to share success stories from 0 sperm, from varicocele repair or mTESE success from those in a similar situation to ours. Thank you so much.
In July my husband had a SA done that showed pretty much low results across the board.
Volume-3.50 / concentration-11.2 / total sperm count - 39.1 / 0% motility. He was previously sick, and our fertility dr advised she’s seen numbers plummet after and to retest in 3 months.
Retest was done on Monday and there was 0 sperm.
Due to his testosterone being so low, and FSH being high - there is no medications they can give to help him at this point.
We have been to a urologist, no blockages found. Testicular failure, and a small pituitary adenoma, that they do not see as an issue.
Has anyone been through a similar situation?
Hi, hoping to hear some experiences around TESA/TESE for idiopathic/non-obstructive severe MFI. (It seems like a lot of the success stories are for obstructive cases.)
We’ve tested everything possible (hormones, chromosomal tests, bacterial cultures, physical factors, etc.) and my husband has been on antioxidant vitamins for 9 months, Clomid for 6 months (testing monthly hormone levels to confirm estradiol and T don’t rise too high), has a great health routine, etc.
10 semen analyses over the past year show an average of 5 million total count, often with no progressive motility, but sometimes 5-10% progressive motility.
DNA fragmentation has been 65 and 70 on two tests 6 months apart.
The two SAs we did at our fertility clinic have shown no motility, so we have been advised by 3 doctors to proceed with testicular sperm. The procedure would begin as a TESA and progress to a TESE if needed.
Has anyone had success in a similar situation?
My husband just got his SA back. I’m assuming this is not the best by any means….our apt is not for another 2-3 weeks to go over results.
Can sauna reduce sperm to zero? Has anyone experienced improvement to “normal” count after stopping sauna?
Hi all, my husband has non obstructive azoospermia due to primary testicular failure and raised FSH. We did microTESE a couple of years ago that combined with ICSI gave us only one embryo which did not implant. We spoke to a professional overseas and he has prescribed anastrazole for 3 months, however my husband is hesitant for the side effects and also because his testosterone levels are normal, and research showed to improve sample for men with low testosterone. Anyone in the same situation that has tried this before with only raised FSH?
I dunno how this post will be received really. A lot of posts on here relate to numbers and results and the like. I don’t know if I’m looking for support, similar stories….i don’t know.
After years of disappointment I’m destroyed by this. We struggled to have our first but nothing like this. All professionals we’ve seen so far have pointed to my high fragmentation as the issue. Ive tried everything to fix it and after a temporary improvement it’s worse than ever. A recent attempt at ICSI with MACS was a disaster. I’m just riddled with guilt and shame that I’m putting my wife through this.
The process has taken something from me I don’t think I’ll get back. It occupies my thoughts constantly, I socialise less….i’m less happy. I look at my friends who have no issues having kids, some overweight, some literally smoke weed all day and others are heavy drinkers. I’ve cut out everything and still no luck. I feel ashamed of being jealous of them. Jealous of random people I see with big families. It’s getting to a point where we’ll have to draw a line under it all and I’m not sure I’ll ever get over the guilt if that happens
Me and my husband are early 30s. We are incredibly lucky already have one child together, conceived naturally, no known fertility issues at the time. We have been trying now for 3-4 years. Recent analysis shows he has azoospermia. We are waiting on his hormone and genetic testing. His ultrasound was all fine. There is no history of illness/drugs/etc. we are completely floored with the results and are really struggling to come to terms with the fact we are unlikely to be able to have more children. We would not be able to afford ivf sadly. Anyone been in a similar situation? Is there any coming back from this? How likely is it the one sample was unrepresentative? Have to wait months for our next results, desperately trying to cling to some hope.
Hello, I did 2 cycles both over 2 years ago. Both were 500test and light anavar. 1 I didn’t do a good pct because I got nervous and stopped 7 weeks in, the other I did after and did everything by the book. Yes I know i was an idiot.
It’s tough finding actual studies on this but can this cause infertility? Most things I read are about guys doing 5-10 years of heavy stuff and TRT. Mine was pretty basic in the grand scheme of roids use. I’m currently waiting on semenalysis results, been over a week. Anyone have any studies or experiences?
Also I did an at home test that said my count was good, obviously those don’t show all the results needed. But is count what would be impacted by trt?

Just wanted to share it with people who can relate. I’m really really really sad about this.
We have been trying actively with my wife for a year. Recently we went to fertility clinic and I just got news that I have low Testosterone and low sperm count. The fertility clinics have referred me to Urologist. I am yet to arrange for that visit. What are the next steps I will undergo to improve the low testosterone and sperm count?
I heard people talking on this sub talking about life style changes. I do not do drugs and don’t drink. What else is required on the lifestyle aspect?

Need your advice.
We are a couple (both 41 yo) with 2 kids (13 and 10) trying for our 3rd. It took us about 18 months to get pregnant with the first one (poor SA for me and temporary no cycles for my SO - now restored). Eventually we succeeded naturally. Had no issues whatsoever the second time. And now TTC for about 2-3 years for the third one - but no luck yet. For my SO we got a Mira test that shows that she doesn't have an LH surge during ovulation (i.e. there is potentially no ovulation, or at least not every cycle). So we are thinking of trying to fix that with Clomid (obviously not by ourselves - aiming to go see a fertility doctor again).
On my side the SA results deteriorated even further (since we were trying for the first one) to the point that a Urologist said "you have no chance with those numbers". I'm a bit more upbeat however - we managed 13 years ago, definetely the chances are not zero now.
I will copy extracts from the urologists letter here:
"He was found to have a few abnormalities on his previous semen analysis, and a more up-to-date semen analysis that was done at TDL, this has revealed 100% abnormal forms (0% normal forms), mainly in the form of sperms displaying vacuolated and pyriform heads with an elevated white blood cell which were identifed as polymorphonuclear leucocytes on the PAP stained slides. He also has a low sperm count of 3.47 mill/ml and a total count of only 8,7 mill sperms. His semen volume was normal at 2.5mL, and the motility also appeared to be normal. An ultrasound scan of the testicles has confirmed only bilateral epididymal cysts with no evidence of varicocele or other testicular abnormalities. His hormonal profile was normal with a testosterone level of 10.7 and a normal FSH level at 3.2 and an LH level of 2.1 and a prolactin of 159. His sex hormone binding globulin was 32. The rest of his bloods, including full blood count, renal functions, and liver functions were reassuringly normal. His cholesterol was borderline raised. He is medically fit and well, and he is not on any regular medications and has no known drug allergies."
So I am now trying to improve my numbers and do another SA in about a 3 months time. From what I gathered so far - supplements I should be taking include Zinc, Coq10 and Fish Oil. Would you recommend anything else? Should I give Ashwagandha a shot - as read mixed things about it? Aparretnly it increases LH but from the letter above mine is normal, so maybe no need? My FHS 20.0 T 240 ng/dl azoospermia ultra sound scanning good doctor prescribed for mtese less than 30% chance. I want to know this type of history effect in future like health condition if so how it will effect.

4 months ago I did a SA with the following result and was diagnosed with mild infertility because of the morphology. But my doctor was not concerned. volume 2.8, concentration 29.4, motile %59, progression 3, morphology 2%. I have since taken fertilaid daily.
This week we had our first IUI appointment with 1x22mm and 1x14mm egg. My PRE wash numbers were: volume 2, concentration 23, motile %70, progression 1, morphology not tested. Post wash was NOTHING left, 0. This was a big surpris.
The next day we gave it another try with a softer wash, knowing that with one day abstinence it’s not gonna be great. PRE WASH: volume 1, concentration 9, motile %59, progression 1, morphology not tested. POST wash: volume 0.5, concentration 1, motile %33, progression 1, morphology not tested.
While the first wash was a total failur, the second softer wash the next day (considering only 1 day) wasn’t in reality any better. About two weeks ago I had food poisoning with a few hours of fewer which could have impacted it.
I could not believe why there would be such an extremely steep drop. But apparently it may have to do with my seamen quality and poor quality that would anyway not make it does not survive the wash? My doctor told me to see an urologist.
Any advise you have would be appreciate!

This is a follow up post from one I did almost a year ago. In short, I was diagnosed with NOA. Urologist suggested to try Chlomid for about 6 months, but SA has consistently shown 0 sperm after a variety of pharmaceutical options.
I had a childhood surgery to fix unilateral cryptochordism and they suspect that it’s all part of the same issue resulting in my inability to produce sperm.
Looking for anyone with similar experiences to get an idea for your journey, your results, experience, and what your recovery looked like following an mTESE.
My most recent bloodwork (pre Chlomid, post):
Estradiol: 54 pg/mL , 54 pg/mL Total Testo.: 646, 682 ng/dL LH: 3.9, 2.4 mIU/mL FSH: 9.9, 6.8 mIU/mL
The urologist mentioned a grade 2 vericocele on one of the testicles, but mentioned testicle volume was average (~ 20 cc) and is optimistic with success from mTESE but I’m not committing to high expectations after the nearly 2 year long journey of disappointing news.

Oligospermia means low sperm count, but my question is when you go to make SA you have to masturbate yourself and i feel like i dont get same sperm out when i do sex with my wife. Can this be one reason for so low count?
25mg clomid, is ED a potential side effect?
I've been taking 25mg clomid for a month now because of suspected low testostrone. I also have been dealing with ED for a year now, I'm 24 years old. I've been taking 2.5 mg daily cialis which seemed to work, this week I started 5 mg daily just because I didn't want to cut it every time. Now, the last 2 times I had sex this weekend I went limp when getting into the top position. This is weird since I've been okay with the cialis.
Is it possible for clomid to cause ED or worsen it? I'm considering lowering the dose or taking it every other day. Let me know your thoughts, thank you.

Hi Community, did anyone having Non Obstructive Azoospermia met with Dr. Peru and had success? I'm having high FSH (24), borderline testosterone, and FNA mapping showed SCO. I saw some old posts about Dr. Peru (Turkey) and his treatment for Azoospermia. If you met with this doctor and had success (sperm found during mtese) then please share your experience. Thanks

Hey y'all. My husband and I are trying to conceive and he got off TRT 8 weeks ago and has been on HCG for almost 4 months. Can any of you share stories that will give me hope? We've ben trying for 3 months but no luck yet obviously... He takes 40 units of HCG 2-3 times a week
