Can I keep the bone removed during surgery?

DJS is still a long way out for me, but I was curious if it would be possible to keep the bone removed during surgery. My surgeon let me keep my molars, so I don’t see why not. Also, weird question, can I ask my surgeon if they can take pictures of me with my maxilla removed? Are they allowed to? I’m a medical student and think it would be super cool.

Anyone else have a dry bottom lip constantly?

I've heard other people have it and it's only on my bottom lip where I don't have much feeling. Is this maybe related to nerve damage? I'm 2 months post op btw

Feeling pretty bummed post up

I was hoping the surgery would add more structure to my face and help stretch some of the soft tissue on my face. I’m about 8 months post op and still have no visible jawline. I was mostly excited for aesthetic improvements so I’m a little down.

Seeking Advice on IVRO vs. BSSO for Lower Jaw Setback

TL;DR: I’m choosing between BSSO and IVRO for my double-jaw surgery, leaning toward IVRO to reduce the risk of permanent numbness. If you’ve had IVRO, how was recovery and stability? Any unexpected drawbacks compared to BSSO? Does IVRO require specific surgeon expertise since it’s less common? Any insights would be much appreciated!

Hey everyone! I’m scheduled for double-jaw surgery soon to correct my underbite and crossbite, and I’m exploring options to avoid long-term complications.

I know this is a long post, but I’m really trying to make the best choice for my upcoming surgery. Any insights, even partial or subjective, would be incredibly helpful. Thanks so much in advance!

A bit of context: The plan is to advance my upper jaw and retract and rotate my lower jaw. My surgeon initially suggested a 3-piece Le Fort I with a BSSO for the lower jaw, but I shared that I already have a paresthesia from a previous wisdom tooth extraction and that I would prefer to explore everything to reduce the chance of another paresthesia. Because of this, my surgeon offered me the option of IVRO (instead of BSSO) for the lower jaw to reduce the risk of permanent numbness (from ~30% with BSSO down to 2-5% with IVRO).

I realize that IVRO would mean my mouth would be wired shut, but I’m okay with temporary limitations if it lowers the risk of permanent complications. I’ve done a lot of reading, and from all the articles I read, none said that IVRO was worse than BSSO in any way apart from the patient discomfort of mouth being wired shut. This keeps me wondering if I am missing something?

Also, I’m quite anxious about this whole process, so any insights from people with experience would be really appreciated.

My questions:

  1. Recovery & Mood: How was your overall recovery after IVRO? I know the first two weeks are likely rough for both BSSO and IVRO, since both involve limited movement initially. But did the wired-shut aspect significantly impact your mood or recovery experience in those first 6 weeks?
  2. Wiring Process: When they wired your mouth shut, did they use the existing braces, or did they add screws or additional elements to secure the upper and lower jaws?
  3. Wire Discomfort: How much did the wires themselves irritate the inner side of your lips? Any tips on managing that?
  4. Technical Aspects of IVRO: I came across a presentation that mentioned IVRO is a “blind procedure” compared to BSSO. Does this mean that IVRO requires more precision or is more complex for the surgeon?
  5. Popularity of IVRO: It seems that BSSO is more commonly chosen for lower jaw surgery. Does this suggest that fewer surgeons have experience with IVRO? Could this lack of familiarity lead to less optimal results or a higher risk of complications?
  6. Long-term Stability: From a stability standpoint, is there a difference between IVRO and BSSO in terms of long-term resilience (e.g., if I were to fall skiing in the future and land on my face, would one method heal in a way that’s more robust for impact)?
  7. Fixation & Healing Process: How does fixation work with IVRO? I assume that after IVRO, the lower jaw is essentially in three parts: two small parts connected to the muscles on the sides, and one large middle section fixated to the upper jaw. What’s keeping everything stable in those early weeks if there’s no rigid fixation, and I assume I will be able to move the outer small parts of the jaw?
  8. Aesthetic Differences: Do BSSO and IVRO produce noticeably different aesthetic results? If so, how would you describe the difference?
  9. Jaw Positioning While Wired Shut: When they wire or band the jaws, do they leave any gap between the teeth to relieve tension, or do the teeth touch? Currently, my teeth don’t touch when I relax my jaw — there’s about 4mm of space between my upper and lower incisors, so I’m concerned about muscle strain if I have to clench to keep the mouth fully shut.
  10. Any Other Drawbacks of IVRO Compared to BSSO? Besides the wiring aspect, are there any drawbacks to IVRO compared to BSSO that I might not have considered?
  11. Any Other Reasons to Prefer One Procedure? Is there something you wish you’d known beforehand, or any reason that led you to prefer one procedure over the other?

(X-RAY) What kind of jaw surgery should I get?

Hi, I've been diagnosed with class 2 malocclusion with 6/7mm overjet and crossbite. I've had almost all of the problems associated with it aside from maybe severe sleep apnea, including:

  • Tmj clicking/popping and pain
  • Gum recession
  • Upper back pain
  • Difficulty sleeping and staying asleep, snoring
  • Bruxism

Because of the Bruxism I also have really damaged molars/premolars, have pain when chewing and cannot close my mouth/stay relaxed or it gets painful quickly.

In 2 days I have a consult with a maxillofacial surgeon through the Italian NHS, so I wanted to be prepared. Are there any questions I should make sure to ask him? Also, what do you think is the best kind of surgery to fix my case?

Swelling, Scar Tissue and Stiffness

What are some of the ways that you guys have dealt with it?

I know of lymphatic massages, mouth exercises and stretches. Is there anything else out there that has helped reduce the three S’s as I called them? (Especially the last two S’s). Thank you!

Learning to chew

I’m making it back to solid foods. 8 weeks after breaking my jaw. So far I’ve tried palak paneer (cheese cubes proved challenging to chew but the rest of it was heavenly), the insides of a samosa, French fries (require more chewing that you’d think so I could only do a few), and scrambled eggs. Chewing food makes me feel like I’ve lifted 200 lbs. with my cheek muscles and I can’t open my mouth very wide which makes utensils challenging. Scrambled eggs were perfection. Need ideas for foods that are about as soft as scrambled eggs and that can go down without chewing.

Dairy post op?

I just got out of palate expansion surgery and also wisdom teeth. They removed four teeth and I believe they cut the top jaw bone. There are a lot of stitches in my mouth rn. I researched and it said to not drink dairy post op but I drank some yogurt on day 2 and day 3 post surgery and there are small bumps under my upper lip and I don’t know if it’s from the dairy cuz I heard it can cause a delay in the healing process?? But idk cuz my nurse said milk was okay but the pamphlet they gave me said I shouldn’t?? And now I’m like hella worried

Jaw surgery with disc displacement

I've struggled for two years now what to do in my case and with my open enrollment going on, I'm thinking about going the other way.

I have one completely asymptomatic displaced disc WITH reduction, and one sometimes symptomatic mostly muscle pain slightly subluxated disc WITH reduction. I don't have severe pain or functional problems. I've had a tolerable open bite my whole life but the subluxation threw my bite off in such a way I finally need jaw surgery.

My ortho is complete and I was scheduled to have arthroplasty with my DJS to have a more "stable" outcome but the longer I sit in support groups, the longer I feel I've yet to see a single positive long term outcome of these disc surgeries and I am now thinking I'd rather take the risk of doing nothing. The jury seems to say well those people weren't good candidates to begin with, discs were too far along but then is it really better to mess with my early stage discs? My jaw movements would be pretty small I was told.

Anyone go through a similar decision tree and how'd it turn out?
